STEP ONE: Submit your intake documents as required below

STEP TWO: Scan all documents, NO PHOTOS, in one email to amha@abbotsfordminorhockey.ca. If you do not receive contact within 3 business days, please contact Rhonda at (604) 832-8295.

STEP THREE: Once all documents have been confirmed, register online.

New players in the U7 and U9 age group (born 2018, 2019, 2020, and 2021) will have the opportunity to register prior to older aged players. ONLY players born in these years are eligible, NO EXCEPTIONS. 

Required documentation:

• Player Registration Form
• Birth Certificate
• Care Card
• 2 proofs of residence (utility bills preferred, must be full first page including tear-off paystub portion)
• Phone bills (solely) of any kind will not be accept as proof of residence
