Name Bars & Letters

All AMHA Name Bars and Letters must follow the established AMHA policy on ordering and application - through AMHA approved seamstress.

AMHA supplies all teams (U11 & Above) with A's and C's at time of jersey pick up.

Name Bars - Orders & Application

Name Bars must be ordered from an approved supplier and applied by an approved professional.

Name Bar Order Form

• AMHA's Apparel committee will complete and process the order with Fraser Valley Cresting.
• After submission of your order, using this form, a representative from the AMHA Apparel committee will be in touch with you within 48 hours. This representative will arrange your jersey drop-off and pick-up time.
• FVC will bill the team directly ($14 per name bar + tax)

Name Bar Seamstress Request

• Once you have picked up your Name Bars from FV Cresting, please submit a request for appointment with the seamstress with the above form
• Please provide a date and time in which you would like to drop off the jerseys. Time with the seamstress is a maximum of 48hrs.
• You can choose to drop off white, red, and black jerseys, or one set at a time. The address will only be provided once the date and time are confirmed.
• AMHA will charge your team the application ($4 per name bar/letter) of the name bars as part of your team transfers.

NOTES FOR DROP OFF:

• Jerseys must be washed - even if only worn once
• ONLY drop off jerseys that will have name bars and/or letters sewn on them (do not include extra jerseys or socks, etc.)
• Jerseys can be dropped off in a labeled bin - make sure in either case the set(s) are labeled with the team name on the top of the bin
• Have a list of jersey numbers with player names for the seamstress inside the bin.

Name Bar Removal

Please complete the Name Bar Removal Request Form. Once completed, you will be contacted to confirm your drop off time and address.
