International Students and Work Visa
International Student
- IIHF Letter of Approval (LOA) (Note: You do not have obtain the signature or approval from your national federation.)
- BC Hockey Declaration of Residence Form
- Letter of support from previous association, if available
- Letter of support from AMHA (provided)
- Letter from parents requesting permission to participate
- Copy of student visa
- Proof of medical coverage
- School registration or confirmation letter from school
- Proof of residence - 2 pieces
- Player Registration Form
- Application for Player Movement (if player played previously)
- Birth Certificate
- Copy of player’s passport
Work Visa
These situations involve non-Canadian parents who are residing in Canada on a long term work permit (e.g. embassy or consulate employees, professional athletes, international corporation employees here for a specific job or time frame). The parents must provide a copy of the appropriate work visa.
- Player Registration Form
- Application for Player Movement (if player played previously)
- Birth Certificate
- Proof of residence - 2 pieces
- If born outside North America: IIHF Letter of Approval (LOA)
- If born in the USA: USA to Canada Letter of Approval for Minor
- Copy of parent’s work visa
- Copy of player’s visa
- Letter of support from New Association (AMHA will provide)
- Letter of support from District (PCAHA) (AMHA will obtain)
